Create custom dialog & custom user config page

We've managed to create a customised design for the dialog panel, but can't replicate the individual cookie user customisation options. Are we able to replicate the customisation check options with the custom HTML template? or is there a shortcode we can use to add this to our details tab?

  The cookie declaration works fine at my end.
