Cookie declaration issue - change/withdraw consent text not showing

I'm new to Cookiebot, so it's very possible that my question will be somewhat naive - forgive me if that's the case..
My cookie declaration preview shows the change/withdraw consent section, as follows:
However, when I load the script onto my web page, the change/consent text does not appear - here is a capture for the web page:
I don't know what is causing this problem - can anyone help? The actual web page is at http://www.galphia.com/about.html.

Hey Phil,
When inspecting your page it seems to me that you have not included the cookiebot's consent dialog script in the head section of your page and that's why when visiting the page I am not asked for my consent. Check step 2 of the installation guide here: https://www.cookiebot.com/en/help/
You have included the cookie declaration script on your page and that's why you are able to view the table with all cookies detected.

I put the cookiebot's consent dialog script in the head section of my index page only - should it be put in the head section of every page on my website?

Yes, it should be on every page you want to get scanned. Usually this is the entire store, so yes, all pages
